Civil Code §4041 requires each owner of a separate interest to provide written notice to the Association of all the following information annually. Please provide the information in the form below and return the completed form to the Association within 30 days. If you do not provide a valid delivery method pursuant to Civil Code §4041, the last mailing address provided in writing by the member or, if none, the property address shall be deemed to be the address to which notices are to be delivered.(Civil Code §4041(c).)
